Student Health Requirements

As stated by the Iowa Department of Public Health

Preschool:

• Current Health Physical (within the last year)
• Immunizations up-to-date – Iowa Immunization Laws and You

Kindergarten:

• Immunizations up-to-date
• Dental Screen (No earlier than age 3 and no later than 4 months after enrollment)
• Vision Screen (No earlier than 1 year prior to and no later than 6 months after enrollment)
• Vision Screen (No earlier than age 3 and no later than 6 months after enrollment)
• Lead Test (At least one test prior to 6 years of age)
• Health Physical (Recommended)

Third Grade:

• Vision Screen (No earlier than 1 year prior to and no later than 6 months after enrollment)

Seventh Grade:

• Tdap and Meningococcal Vaccine #1 (See above link for immunization schedule)

Ninth Grade:

• Dental Screen (No earlier than 1 year prior to and no later than 4 months after enrollment)

Twelfth Grade:

• Meningococcal Vaccine (Dose #2)

Healthy Snack Options

Crackers, Goldfish, Pretzels, Cheez-Its, Popcorn, Graham Crackers, Rice Cakes

Yogurt, Fruit Cups, Fruit/Veggie Pouches, Fresh Fruit or Veggies, Applesauce

Cheese Bites/String Cheese, Olives, Meat Sticks

Please be mindful of nut allergies in our school district. Food items containing nuts or that have been processed in a facility where cross-contamination may occur are not allowed. We request that celebration treats be individually packaged with the food ingredients attached. No homemade products are permitted.
